Summary
By measuring intraocular pressure in different body positions from 60° semiupright to 30° head down, a nonlinear relationship between IOP increase and body position was confirmed. IOP postural response in individual subjects was roughly correlated to ophthalmic arterial pressure and to the episcleral venous pressure postural response. In one series of subjects, the episcleral venous pressure increments due to posture were parallel to the applanation-indentation disparity in the same individual eyes. Differential tonometry with applanation or indentation procedures under blind conditions gave significantly low indentation readings. It is concluded that IOP postural response depends on arterial and venous vascular changes when subjects move from an erect to a horizontal body position. Blood expulsion from the choroid by indentation tonometry might be the reason that this tonometric procedure does not measure IOP changes based on vascular changes.
Zusammenfassung
Die Messung des Augendruckes in verschiedener Körperlage zwischen 60° halb-aufrecht und 30° Kopftieflage ergab einen nicht-linearen Zusammenhang von Körperlage und Augendruckanstieg. Die Änderungen des Augendruckes waren gleichsinnigen Änderungen des episkleralen Venendruckes und des Ophthalmikablutdruckes korreliert. In einer weiteren Untersuchungsreihe wurde die Parallelität von Applanation-Indentations-Unterschied und den Effekten des episkleralen Venendruckes bei Änderung der Körperlage untersucht. Die Differentialtonometrie von Applanation und Indentation unter strengen Blindbedingungen ergab signifikant niedrigere Werte mit der Indentationstonometrie. Es wird geschlossen, daß der Augendruckanstieg bei Lagewechsel mit synchronen Änderungen des venösen und arteriellen Kreislaufs einhergeht. Die Verdrängung von Blut aus der Choroidea durch die Indentationstonometrie erklärt die Tatsache, daß mit dem Schiötz Tonometer Augendruckänderungen aufgrund von Durchblutungsänderungen wie bei Lagewechsel übersehen werden.
