Grading of posterior capular opacification (PCO)

PCO grade Definition
Grade 0Clear posterior capsule
Grade 1PCO not seen in the central visual axis.
A clear view of the optic nerve head and retinal nerve fibre layer and blood vessels is obtained
Grade 2PCO seen in the central visual axis with mild obscuration of fundus detail: optic nerve head is clearly seen but retinal nerve fibre layer and blood vessels are blurred
Grade 3PCO in the central visual axis with marked blurrring of fundus details: the margins of the opric nerve head is not clearly defined
  • Patients are examined with a undilated pupil using a direct ophthalmoscope.
