Obtaining Auricular Cartilage for Reconstructive Surgery

The cartilage in the scaphoid fossa between the helix laterally and the antihelix medially provides excellent underlying support for soft-tissue eyelid and nasal reconstruction. After subcutaneous infiltration anesthesia is administered through both the anterior and posterior auricular skin, an incision is made along the posterior rim of the helix. A dissection plane between the perichondrium and skin exposes the cartilage. The cartilage is removed without incising the anterior skin surface. This technique is particularly useful in cicatricial entropion, upper or lower eyelid retraction, eyelid reconstruction, and socket reconstruction.

    This is a simple graft to harvest, with the eyelid anesthetized and everted and the tarsus harvested from the upper edge of the tarsus, keeping 2.5 mm to 3 mm of marginal tarsus undisturbed. If there is no good option for a tarsal graft, hard palate mucosa,16 nasal chondromucosa, ear cartilage,17 and commercially available tissue grafts18 can be substituted. A lid margin laceration or defect extending medial to the puncta, a canalicular laceration, or defect must be suspected.

    Upper eyelid retractor release does not require the placement of a spacer, but lower eyelid retractor release typically does and this may be provided by ear cartilage graft, hard palate graft, or a synthetic alternative. Our preference is ear cartilage as it causes less discomfort to the patient than hard palate grafts and, when harvested well by avoiding the rim of the ear, leaves an imperceptible cosmetic deficit.58 Procedures designed to address eyelid retraction should take place after strabismus surgery since the latter may exacerbate eyelid retraction.
