*The CSR reflects the number of surgeries performed in 1 year per million population. Our study sample size was 24 327 (male 11 889, female 12 438). We calculated the CSR as follows: CSR=(number of surgeries reported in study in 12 months×1 000 000)/study sample size. Example calculation: overall CSR=(221×1 000 000)/24 327=9085.
