Zusammenfassung

Hintergrund Bei der modernen nicht-perforierenden Glaukomchirurgie wird die Integrität des Trabekelmaschenwerks bewahrt und eine tiefe Skleralamelle reseziert. Ziel unserer Studie war es, die Beziehung zwischen der Morphologie des tiefen Skleraresektats und dem postoperativen Augeninnendruck zu untersuchen.

Methodik Bei 17 Patienten mit adultem Offenwinkelglaukomen wurde als antiglaukomatöser Ersteingriff eine Viskokanalostomie durchgeführt und das tiefe Skleraresektat licht-mikroskopisch untersucht. Retrospektiv wurde der postoperative Verlauf des Augeninnendrucks mit der Morphologie des tiefen Skleralappens qualitativ (Nachweis von Anteilen des Trabekelwerks oder des Schlemmschen Kanals) und quantitativ (Dicke des Skleralappens) korreliert.

Ergebnisse Der maximale Augeninnendruck sank durch die Viskokanalostomie von präop. 36,2 ± 8,5 mm Hg auf 10,6 ± 5,7 mm Hg am 1. postop. Tag und 12,4 ± 4,9 mm Hg am 3./4. postop. Tag. Nach einem Nachbeobachtungszeitraum von 6 Monaten im Median lag der Augendruck bei 21,7 ± 5,5 mm Hg. Weder die Dicke des tiefen Skleralappens (im Mittel 309 ± 95 μm) noch das Präparationsniveau (5 Skleralappen mit Trabekelmaschenwerksgewebe, 6 ohne Zeichen einer Entdachung des Schlemmschen Kanals) zeigten einen statistisch signifikanten Zusammenhang mit dem postoperativen Augendruckverhalten.

Schlussfolgerungen Die lichtmikroskopisch nachweisbaren Schwankungen bei der Präparation der tiefen Skleralamelle scheinen das Augeninnendruckniveau in der frühen postoperativen Phase wenig zu beeinflussen. Andere Faktoren, z. B. die Nahtfixation der äußeren Skleralamelle oder unerkannte Mikrorupturen im Bereich des Trabekelwerks bzw. der Descemet-Membran, sind möglicherweise für den früh-postoperativen Augendruck von Bedeutung.

Background Non-perforating glaucoma surgery preserves the integrity of the trabecular meshwork in order to avoid postoperative hypotony. The purpose of our study was to investigate whether the morphologic variability of the excised deep scleral flap influences the postoperative intraocular pressure (IOP) after viscocanalostomy.

Methods Light-microscopy of the deep scleral flap was performed in 17 patients who had undergone viscocanalstomy. Morphologic parameters (thickness and dissection level of the deep scleral flap) were correlated with the postoperative IOP.

Results The mean thickness of the deep scleral flap was 309 ± 95 μm; the dissection level was too deep in 5 cases (trabecular tissue excised) and too high in 6 cases (no signs of Schlemm's canal). The max. preop. IOP was 36.2 ± 8.5 mm Hg and came down to 10.6 ± 5.7 mm Hg at day 1 postop. and 12.4 ± 4.9 mm Hg at day 3/4 postop. At a median follow-up of 6 months IOP was 21.7 ± 5.5 mm Hg. A significant correlation between postop. IOP and the morphology of the deep scleral flap could not be demonstrated.

Conclusions Variations of the thickness and depth of the deep scleral flap showed little influence on the initial IOP level following viscocanalostomy. Other factors, f. e. the suturing of the external flap or invisible microruptures of the trabecular meshwork, could be of importance for the early postoperative IOP.
