Zusammenfassung

Hintergrund Die Refraktionsbestimmung von unkooperativen Patienten stellt für Augenärzte eine große Herausforderung dar. Die dynamische Skiaskopie liefert zwar in erfahrenen Händen genaue Ergebnisse, erfordert jedoch Übung und Zeit. Das Videorefraktometer VRB 200 (Firma Tomey, Erlangen) wurde besonders zur Refraktionsmessung von Kindern entwickelt. Diese Untersuchung sollte klären, welche Genauigkeit das Gerät bei der Refraktionsbestimmung erreicht.

Material und Methoden 259 Patienten (Alter 10 Monate bis 77 Jahre) wurden mit dem VRB 200 gemessen. Jeder Patient wurde in Zykloplegie skiaskopiert und am Topcon-Autorefraktometer RM-A 6000 refraktioniert. Bei 5 emmetropen Probanden erzeugten wir mit Hilfe von Schmalrand-Probiergläsern Ametropien. Diese Messreihen führten wir mit und ohne medikamentöse Zykloplegie durch.

Ergebnisse Beim VRB 200 war der Mittelwert des Fehlers des sphärischen Äquivalents bei Fehlsichtigkeiten von - 4 bis + 4 Dioptrien (dpt) < 2 dpt. Bei stärkeren Fehlsichtigkeiten wuchs der Fehler zunehmend. Wiederholte Messungen eines Patienten ergaben starke Schwankungen der Ergebnisse. Astigmatismus wurde mit zunehmender Größe schlecht erfasst, besonders bei schräger Achslage (45o und 135o). Anisometropien > 2 dpt wurden nur zu 72 % erkannt. Es konnten viele Kleinkinder und geistig retardierte Patienten mit dem VRB 200 refraktioniert werden, die mit dem Topcon-Autorefraktometer RM-A 6000 nicht zu untersuchen waren. Eine Refraktionsbestimmung mit dem Skiaskop war in allen Fällen möglich, auch wenn dies viel Zeit und Geduld beanspruchte.

Schlussfolgerungen Das Konzept der guten Handhabbarkeit leidet besonders bei Ametropien jenseits ± 4 dpt an zunehmender Ungenauigkeit. Bei dem momentanen Entwicklungsstand stellt das VRB 200 im praktischen Einsatz keine Alternative zur konventionellen Skiaskopie dar.

Background Measurement of refraction of non-cooperative patients is a real challenge for the ophthalmologist. Retinoscopy produces exact results in experienced hands, but requires practice and time. The Videorefractor VRB 200 (manufactured by Tomey, Erlangen) was especially designed for refracting children. The aim of this study is to prove the accuracy of this tool in measuring refraction.

Material and Methods 259 patients (age 10 month to 77 years) were measured with the VRB 200. Each patient was examined by retinoscopy in cycloplegia and refracted with the Topcon autorefraktor RM-A 6000. Ametropia was evoked at 5 emmetropic volunteers by lenses of different power, placed in a trial frame. These measurements were performed with and without use of cycloplegic drugs.

Results The VRB 200 showed an average error of spherical equivalent at ametropia of the range - 4 to + 4 Diopters (dpt) < 2 dpt. At higher ametropia the error grew increasingly. Repeated measurements of one patient showed high variation of the results. Astigmatism was measured worse by growing magnitude, especially at diagonal axis (45o and 135o). Anisometropia > 2 dpt has been discovered in only 72 %. It was possible to examine many infants and handicapped patients, which was impossible by using the RM-A 6000. An examination by retinoscopy was possible in all cases, but needed a lot of time and patience.

Conclusions The concept of good handling suffers especially by the increasing inaccuracy at ametropia beyond ± 4 dpt. At this state of development the VRB 200 is no alternative in practical use to the conventional retinoscopy.
